I need a simple code that I can duplicate.
Using clickatell.com I want a form to be completed and the form to be sent to two mobile numbers.
Form consists of
Contact Person (must be completed)
Contact Number (must be completed)
Email Address (optional to complete)
Text Area (optional to complete)

Optional Extra: A pre-populated message must be sent to the number completed in the form.
